Get Your Requisition Form
Before your appointment you will need your physician to fill out a laboratory Test Requisition Form.
Make an Appointment or Walk-In
Make sure to have your signed Requisition Form the day of your test.
Get Your Results
In most cases, test results will be available within 1-2 days. Depending on state regulations, results will be sent via e-mail, fax, or mail. For New York residents, we are required to send test results to doctor.
Speak to Your Doctor
Once you have your test results, you should discuss them with your doctor. If you need further laboratory testing, please come see us again.
